This is a miniature of a grick, a wormlike creature from Dungeons and Dragons.

Gricks hide in caves and surprise passing prey by suddenly attacking them and revealing their mouth and tentacle like arms to grab it. If food gets scarce they also lurk above ground in forests. The big gricks are well fed and called alpha gricks and they usually lead a pack of normal gricks.
